If necessary, you should print this document and read it while you are on your computer. Click here to print this document.

You can print USBA documents such as:

1) USBA Membership Applications
2) How to Play 3-Cushion Billiards (for those that don’t know the game)
3) Membership Advertisements (to hand out to non-members)
4) Custom Room Membership Applications (unique for your billiard room)

Look at the left-hand menu of options on the USBA website (www.USBA.net).

The 12th option from the top says, “USBA FORMS”. Click that option.

You will then be able to open a PDF file which can be printed for your use.

Custom Room Membership Forms: If you would like to print Membership Applications with the name of your room already pre-printed on it, then click this option. Then scroll down to find the state where your room is located. Then find your room. You can then open and print either a PDF or a Microsoft Word file of the Membership Application. If your room is not included in the list, then please contact the secretary right away by clicking “Contact Us” on the left side menu.

If you would like to receive a USBA Countertop Membership Recruitment Display and a Poster for your room, then contact the secretary by clicking “Contact Us” on the left side menu.
